for Training Form
The ongoing HAZWOPER
courses in Tampa, Florida are held each month
on the following days:
The first four consecutive days (Monday thru Thursday)
of each month are generally reserved for the 40-Hour
HAZWOPER (bumped one week later if a
holiday falls in this schedule). The last of the
four days (Thursday) will include any extra students
for the 8-Hour HAZWOPER refresher.
Other
arrangements can be made for classes outside of the
regular training schedule. Please fill in the
form below and submit a request for a training date
which will fit your schedule. NETI Inc. will
respond to you with a quote for the desired training.

Terms
and conditions:
Upon
completion of the above form, NETI Inc. will
contact you with a quote for the training or a
request for more information. Upon
acceptance of the quote, your company will be
faxed a "Contract for Services"
which will be signed by an authorized
representative to reserve the training date
for your company. An invoice will be
faxed to your company and the advance
registration fee of 25% of the contract price
will then be due to confirm the date.
The balance of the contract price is due on
the first day of training. If a
re-schedule is needed from the reserved date,
10% of the total contract price is required
for the registration date to be changed.
No advance registration fees are refundable
upon cancellation.
Each
person attending the course will receive a
full-sized, wall certificate of training and a
personal laminated wallet picture ID card for
on-site verification of training purposes.
A list of students to attend with their SS#
can be E-mailed ahead of time to assist in a
timely processing of the certificates and ID
cards. The attendees, upon completion,
will be issued an OSHA tracking number
good anywhere.
